To sign up for a new membership:
- Fill out this form and click the Submit button (this will create a printable page).
- Print the application and waiver agreement on the printable page.
- Sign the waiver (all members must sign).
- Mail a check or money order made out to SFCC for the amount selected (no cash please) and application & waiver to:

To renew or update your membership:
- Enter your membership name and e-mail address on this form.
- Include any information that may have changed from your previously submitted application.
- Click the Submit button, print, and mail the form to the above address.
- Note: any new family members must sign the waiver.
